MEMBER CONSENT, DATA AUTHORISATION & SERVICE AGREEMENT

1. INTRODUCTION AND DEFINITIONS

This Member Consent ("Consent") is a binding agreement between you ("Member", "you") and Longaevitas Health (Pty) Ltd (Registration Number 2025/413084/07), a private company incorporated in the Republic of South Africa with its registered office at 18 Kent Road, Dunkeld West, Randburg, 2196 ("Longaevitas", "we", "us", "our"). In this Consent: "Services" means the Longaevitas preventative healthcare and longevity-planning platform, including the Health Assessment, VitaPrint Cycles, Doctor-led VitaPrint Analysis, educational supplement guidance, partner information, and any associated content or communications. Longaevitas does not sell, supply, dispense, ship or fulfil supplements or any other health product; "VitaPrint" means the personalised preventative healthcare record generated for you within the Services; "Practitioner" means a medical doctor registered with the Health Professions Council of South Africa ("HPCSA") who reviews your information through the Services; "Personal Information" and "Special Personal Information" have the meanings given in the Protection of Personal Information Act 4 of 2013 ("POPIA"); "ECTA" means the Electronic Communications and Transactions Act 25 of 2002; "CPA" means the Consumer Protection Act 68 of 2008; and "National Health Act" means the National Health Act 61 of 2003. This Consent must be read together with the Longaevitas Privacy Policy, the PAIA Manual, and the applicable HPCSA ethical guidelines (including the Booklets on General Ethical Guidelines for the Healthcare Professions and on Telemedicine). To the extent of any inconsistency between this Consent and any other Longaevitas policy, this Consent prevails for matters concerning the use of the Services and the clinical relationship. By acknowledging each section below, signing the final declaration electronically, and clicking Accept and Submit, you confirm that you have read, understood and accepted every clause of this Consent. Pursuant to section 13 of ECTA, your electronic acknowledgement and signature constitute a valid signature with the same legal effect as a handwritten signature.

2. NATURE OF THE SERVICE

Longaevitas provides a doctor-led, preventative healthcare and longevity-planning platform. The Services include: preventative health screening; longevity planning; health optimisation; identification of modifiable risk; personalised lifestyle, nutritional and supplementation guidance; and the doctor-led interpretation of health information you supply. Longaevitas is an information and planning platform: it does not sell, supply or deliver supplements or other health products, and any product you choose to purchase is bought from an independent retailer under that retailer's own terms. The Services are designed to assist healthy and clinically stable adults to reduce future health risk through preventative measures. The Services are not intended to diagnose, treat, cure or manage acute illness; they do not replace the assessment, examination or treatment provided by your treating medical practitioners; and they do not constitute emergency medical care. Information shared through the Services, whether in your VitaPrint, written correspondence, or supporting educational material, is provided for your personal use as part of a doctor-led preventative programme and may not be relied upon for any other purpose without independent clinical advice.

3. NO EMERGENCY SERVICES

Longaevitas is not an emergency service, an ambulance service, a hospital, a clinic, an after-hours service, or a crisis service. The Services are asynchronous and there is no guarantee of immediate response by any Practitioner or member of the Longaevitas team. If you experience, suspect, or are at risk of a medical emergency, you must immediately telephone your nearest emergency medical service (in South Africa, 112 from a mobile telephone or 10177 from a landline), contact your treating medical practitioner, or attend the nearest emergency department. You must not wait for, or rely upon, any communication through the Services in an emergency.

4. DOCTOR-LED CLINICAL CARE

Clinical recommendations issued through the Services are produced under protocols, reference ranges and rules set and maintained by HPCSA-registered Practitioners. An HPCSA-registered Practitioner exercises independent clinical judgement and remains responsible for the final clinical content of your Doctor-led VitaPrint Analysis. Automated processing and platform assistance. Longaevitas uses automated processing, including rule-based scoring and artificial intelligence, to organise your information, calculate system scores, disease-risk estimates and your Longaevitas Age, to draft educational content, and to prepare a structured briefing for the Practitioner. These outputs are preparatory and educational. They are not clinical decisions. Two release paths apply. Where a result falls into the abnormal band, or an escalation, adverse event or referral flag is raised, a Practitioner personally reviews, amends where necessary and approves your analysis before it is released to you. Where no result falls into the abnormal band and no safety flag is raised, your analysis may be released automatically under the Practitioner-set protocols described above, in the name of the clinical head of your allocated care team, without that Practitioner reading it individually first. A random sample of these automatic releases is reviewed afterwards by that clinical head for clinical quality. For the purposes of section 71 of POPIA, you have the right to ask that a Practitioner individually review and reconsider any output released to you, to ask how it was arrived at, and to make representations about it. Longaevitas will provide that individual Practitioner review on request when you escalate a question to a doctor in Ask Vita in your member portal. Each VitaPrint cycle includes a free doctor escalation, so this review costs you nothing. Automated outputs are educational and preparatory and do not diagnose, prescribe or treat. Where appropriate, the Practitioner may decline to issue a recommendation, may require additional information, or may recommend that you consult a treating medical practitioner in person.

5. CLINICAL LIMITATIONS

I acknowledge and accept the following clinical realities: (a) No health assessment, biomarker panel, screening tool or risk score can detect every disease, latent condition, or future event. Many conditions cannot be diagnosed without a full in-person history, physical examination, imaging, or additional investigations. (b) All risk scores, ranges, projections and longevity estimates are probabilistic and are derived from population-level evidence. They describe likelihood across a population and do not predict the future for any individual with certainty. (c) Preventative care reduces risk; it cannot eliminate disease, ageing, injury or death. Adverse health events may occur despite optimal preventative care. (d) A result reported as normal does not exclude existing or future disease and does not guarantee future health. Conditions may develop or change between assessments. (e) All recommendations issued through the Services are based on the information available to the Practitioner at the time of review. New information, results, symptoms or circumstances may render an earlier recommendation incomplete or inappropriate.

6. MEMBER RESPONSIBILITIES

The safety, accuracy and value of every recommendation issued through the Services depend on the information you provide. I undertake that, to the best of my knowledge: (a) all information I supply is true, complete, current and unaltered, and I will not omit information that may be material to my health; (b) I will disclose all prescription and over-the-counter medications I am taking, including dosages, frequencies and recent changes; (c) I will disclose all supplements, vitamins, herbal preparations and recreational substances I am taking; (d) I will disclose all known allergies and adverse reactions, particularly to medications, supplements or foods; (e) I will disclose all current and previous diagnosed medical conditions, mental health conditions, and any condition under active investigation or treatment; (f) I will disclose previous surgeries, hospital admissions, and significant medical events; (g) I will disclose relevant family medical history where it is known to me; (h) I will disclose pregnancy, planned pregnancy, breastfeeding, fertility treatment or any other relevant reproductive status where applicable; (i) I will promptly inform Longaevitas of any material change in my health, medications, treatments or circumstances; and (j) I accept that inaccurate, incomplete or out-of-date information may render a recommendation unsafe or inappropriate, and the Practitioner is entitled to rely on the information I have provided.

7. BLOOD RESULTS AND EXTERNAL INVESTIGATIONS

Where biomarker results, imaging, genetic results or other investigations are uploaded to or generated through the Services, I acknowledge that: (a) all laboratory and diagnostic testing has technical, biological and analytical limitations, including measurement variability, reference-range differences between laboratories, pre-analytical influences (such as fasting, hydration, time of day, recent illness, exertion or medication) and the inherent uncertainty of any test; (b) results reflect a single point in time and may change with health status, treatment, diet, life events or natural variation; (c) Longaevitas does not perform the laboratory or diagnostic testing itself; testing is performed by independent third-party laboratories and providers, and Longaevitas is entitled to rely upon, but does not warrant, the accuracy, completeness or timeliness of results supplied by those providers; (d) recommendations are based only on the results actually available to the Practitioner at the time of review; and (e) abnormal, unexpected or borderline results may require further investigation, repeat testing, or assessment by a treating practitioner; I will follow such recommendations promptly when given; and (f) where I have obtained a laboratory request form from Longaevitas and my results have not been loaded onto the platform, the Longaevitas operations team may contact me on the mobile number I provided, may receive my results directly from the laboratory or from me, and may load those results onto my VitaPrint Cycle on my behalf so that a Practitioner can complete the analysis. This operational assistance is part of the Services and is recorded in my clinical record.

8. LIFESTYLE RECOMMENDATIONS

Lifestyle recommendations issued through the Services in the fields of nutrition, exercise, sleep, supplementation, stress management, weight management, performance optimisation and longevity are educational and preventative in nature. Implementation of any lifestyle recommendation is voluntary and is my responsibility. I will introduce changes with appropriate gradation, will discontinue any change that causes adverse effects, and will obtain in-person clinical advice before making changes that may interact with existing conditions, medications, pregnancy or planned pregnancy. No specific health, body composition, performance or longevity outcome is promised or guaranteed by Longaevitas. Individual responses to lifestyle interventions vary widely.

8A. VITA, WHATSAPP AND SELF-LOGGED RECORDS

By accepting this consent, I acknowledge and agree that:

  • Vita is an educational wellness assistant and my longevity co-pilot. She is not a doctor, not a diagnostic tool and not a medical device, and she does not diagnose, prescribe, triage or monitor any condition.
  • Using Vita on WhatsApp is my choice. Messages and any media I send are processed on Meta Platforms infrastructure outside South Africa, as described in the Privacy Policy, and I consent to that processing.
  • WhatsApp is not monitored in real time and is never to be used for emergencies. In an emergency I will contact my nearest emergency service.
  • I consent to receiving service messages from Vita on WhatsApp, such as weekly plan check-ins and reminders I asked for, and I understand I can pause or stop them at any time.
  • Nutrition figures, extracted label and report values, and any other figure calculated from what I send are estimates. My logs, medicine list, supplement list and appointment notes are my own self-reported records, and I am responsible for checking they are correct.
  • Interaction and timing notes about food, medicines and supplements are general education from doctor-developed rules, not a clinical medication review, and I will raise medicine questions with my prescribing doctor or pharmacist.
  • Reminders and held documents are a convenience. Attending my appointments remains my responsibility, and a document held for my next cycle is not reviewed as an urgent clinical alert.

9. SUPPLEMENT GUIDANCE

Longaevitas provides supplement guidance as information. Longaevitas does not sell, supply, dispense, compound, ship or fulfil any supplement, and does not hold stock. Where a retailer or wellness partner is named alongside a suggested active ingredient, that information is provided for convenience only. Any purchase is a separate transaction between me and that independent retailer, on that retailer's own terms, and Longaevitas is not a party to it. Supplements suggested through the Services are intended as adjuncts to a healthy lifestyle and a doctor-led preventative programme. The decision to take, continue or discontinue any supplement is mine to make. Individual responses to supplements vary. Supplements may interact with prescription medication, may be inappropriate during pregnancy, breastfeeding or specific clinical conditions, and may produce adverse effects. I will discuss any concern, side effect or interaction with my treating practitioner before continuing. Product quality, labelling, safety, availability, pricing, delivery and regulatory compliance under the Medicines and Related Substances Act 101 of 1965, the Foodstuffs, Cosmetics and Disinfectants Act 54 of 1972 and the applicable South African Health Products Regulatory Authority requirements remain the responsibility of the manufacturer and the retailer from whom I purchase. Longaevitas makes no therapeutic claim beyond the educational information presented in my plan.

10. REFERRAL RECOMMENDATIONS

From time to time the Practitioner may recommend that I consult one or more other healthcare professionals, including general practitioners, medical specialists, dietitians, biokineticists, psychologists, physiotherapists or other allied health practitioners. Referral recommendations are made for my benefit. Choice of provider, the booking of any consultation, attendance, payment and the conduct of the consultation are my responsibility. Longaevitas does not assume responsibility for, and is not vicariously liable in respect of, the care, advice, treatment, charges, availability or conduct of any external healthcare professional whom I consult, whether on referral or otherwise.

11. MEMBER ACKNOWLEDGEMENTS

I confirm my understanding that: (a) preventative care can meaningfully reduce risk but cannot eliminate disease, ageing or death; (b) clinical guidance evolves as scientific evidence changes, and recommendations issued today may be revised in future iterations of my VitaPrint; (c) long-term, periodic monitoring and re-assessment is materially more beneficial than any single assessment in isolation; and (d) no guarantee is made, expressly or by implication, regarding any specific health outcome, biomarker change, longevity benefit or absence of future disease.

12. CONSENT FOR CLINICAL COMMUNICATION

I consent to receive the following clinical and service communications from Longaevitas and the Practitioner through secure electronic means (including in-platform messaging, email, SMS and PDF attachments accessible through my account): my VitaPrint and any updates thereto; doctor-led recommendations; educational material relevant to my care; laboratory and investigation requests; referral letters; appointment, follow-up and reminder communications; and other clinical follow-up communications related to my care. I understand that electronic communication is not perfectly secure; Longaevitas applies reasonable technical and organisational measures (encryption in transit and at rest, access control, audit logging) but cannot eliminate every risk associated with internet-based communication. I may at any time request that a specific communication be delivered in an alternative form.

13. DATA PRIVACY (POPIA)

Longaevitas is the responsible party for the processing of your Personal Information and Special Personal Information (which includes your health information) under POPIA. Lawful basis. We process your information on the lawful grounds of your express consent, the performance of this Consent, compliance with legal obligation, and our legitimate interest in operating the Services safely and securely. What we collect. Identification and contact details, demographic information, account preferences, assessment answers, body measurements, biomarker results, uploaded laboratory reports, payment metadata, and technical or usage data. Why we process. To provide the Services; to enable the Practitioner to issue a Doctor-led VitaPrint Analysis; to generate your VitaPrint and recommendations; to manage referrals, payouts and account administration; to send clinical and service communications; and to comply with legal, regulatory and professional obligations. Sharing. We share Personal Information only with: the Practitioner reviewing your information; healthcare professionals directly involved in your care, on your instruction; laboratories and imaging providers involved in your care; payment partners; secure technology providers acting on our behalf under written processing agreements; and regulators or law-enforcement agencies where required by law. We do not sell your Personal Information. Cross-border processing. Where Personal Information is processed outside the Republic of South Africa, we apply the safeguards required by section 72 of POPIA. Security. We implement appropriate technical and organisational measures to safeguard your information, including encryption in transit and at rest, access control, role-based permissions, audit logging, staff confidentiality undertakings and regular review. Retention. Your health-related information is retained for the period required to operate the Services and to comply with the National Health Act, HPCSA guidance and other applicable law. Standard retention is the longer of (i) six years after your last interaction with the Services and (ii) any longer statutory period, including the extended periods that apply to records of minors and of persons under legal disability, and to records relevant to a claim, complaint or investigation of which we are aware. Identifiers are removed once the retention period lapses, save where ongoing retention is required by law. Security compromises. If a security compromise occurs in which there are reasonable grounds to believe your Personal Information has been accessed or acquired by an unauthorised person, we will notify the Information Regulator and you as soon as reasonably possible after discovering the compromise and establishing its scope, in accordance with section 22 of POPIA. The notification will describe the possible consequences, the measures we have taken or intend to take, and what you can do to reduce any adverse effects. Your rights. You have the right to access, correct, delete, object to or restrict the processing of your information, to data portability where applicable, to withdraw your consent (subject to the legal and clinical consequences of withdrawal), and to lodge a complaint with the Information Regulator (South Africa) at JD House, 27 Stiemens Street, Braamfontein, Johannesburg; complaints.IR@justice.gov.za. 13A. VITA COHORT LEARNING AND DE-IDENTIFIED INSIGHTS

Vita is the Longaevitas platform assistant. To keep educational guidance current and relevant, Longaevitas analyses de-identified trends across the member community, including changes in assessment answers, body metrics, biomarker values, and outcomes following supplement or plan recommendations. This analysis produces cohort-level insights that inform the educational content Vita presents to members and the protocols doctor-led practitioners choose to consider. It does not diagnose, prescribe or replace clinical judgement. Before your information is used for this cohort learning, it is de-identified (direct identifiers such as name, contact details and account identifiers are removed) and only aggregated results are used. No individual member is identifiable in the cohort insights that reach Vita, doctors, staff or any downstream user. Longaevitas will not sell your information and will not use it to train third-party artificial intelligence models. Opt-out. Cohort learning is on by default when you join. You may withdraw at any time from Account > Privacy, and your future data will no longer be included in cohort learning. Withdrawal does not affect the doctor-led clinical care you receive through the Services, and cohort insights already generated from historic de-identified data cannot be reversed once aggregated. Educational, not clinical. Cohort insights are educational proxies. They inform what Vita may surface and what doctors may consider, but every clinical decision remains with an HPCSA-registered Practitioner and, ultimately, your own treating doctor.

14. CLINICAL RECORDS

I consent to Longaevitas maintaining comprehensive clinical records of my interactions with the Services, including my Health Assessment answers, biomarker results, VitaPrint Cycles, doctor-led analyses, communications, referrals and any associated audit data, in accordance with the National Health Act, the HPCSA ethical guidelines on the keeping of patient records, and POPIA. Clinical records are retained for the periods required by law and applicable professional guidance. Subject to the National Health Act and POPIA, I have the right to request a copy of, or correction to, my clinical records by contacting support@longaevitashealth.com.

15. THIRD-PARTY PROVIDERS

The Services depend on independent third-party providers, including pathology laboratories, sample-collection partners, imaging providers and specialist healthcare professionals. While Longaevitas selects partners with reasonable care, it does not control their day-to-day operations. Longaevitas is not responsible for, and accepts no liability arising from, delays, errors or service interruptions on the part of any laboratory, specialist or other third-party provider, or for the unavailability of any external service. Where such an issue occurs, Longaevitas will use reasonable endeavours to assist in resolving it.

16. LIMITATION OF LIABILITY

This clause is drawn to your attention in accordance with section 49 of the CPA. Please read it carefully. It limits the liability of Longaevitas and its associated persons. To the maximum extent permitted by law, Longaevitas, its directors, officers, employees, contractors, the Practitioners, affiliates, secure technology providers and partner organisations (collectively, the "Indemnified Parties") shall not be liable for: (a) any indirect, incidental, special, consequential, exemplary or punitive loss or damage; (b) loss of profits, loss of revenue, loss of opportunity, loss of data or loss of goodwill; (c) any loss or damage arising from your failure to disclose accurate, complete or current information; (d) any loss or damage arising from your failure to seek emergency or other in-person medical care when indicated; (e) any loss or damage arising from the acts, omissions, delays, errors or service interruptions of any third-party provider; or (f) any loss or damage arising from your reliance on the Services contrary to the warnings contained in this Consent. The total aggregate liability of all Indemnified Parties, taken together, arising from or in connection with the Services, regardless of the form of action, shall not exceed the greater of (i) the aggregate amounts actually paid by you to Longaevitas in the twelve (12) months immediately preceding the event giving rise to the claim and (ii) ZAR 5 000 (five thousand Rand). Nothing in this Consent excludes or limits liability that cannot lawfully be excluded or limited under South African law, including liability for fraud, intentional wrongful conduct, gross negligence, death or personal injury where liability cannot lawfully be excluded under the CPA or the common law, or any other statutory liability that cannot be excluded.

17. INDEMNITY

To the maximum extent permitted by law, I indemnify and hold harmless the Indemnified Parties (as defined above) from and against all claims, demands, actions, losses, damages, costs and expenses (including reasonable legal costs on the attorney-and-own-client scale) arising directly or indirectly out of: (a) inaccurate, incomplete or untrue information I provide through the Services; (b) my failure to disclose material health information; (c) my failure to seek appropriate or emergency medical care; (d) my use of the Services contrary to the warnings contained in this Consent or in any communication issued to me by the Practitioner; or (e) my breach of any provision of this Consent. This indemnity does not apply to the extent that the loss arises from the fraud, gross negligence or wilful misconduct of an Indemnified Party.

18. INTELLECTUAL PROPERTY

All intellectual property in the Services - including reports, VitaPrints, risk scoring, educational material, clinical frameworks, decision logic, assessment methodology, written content, graphics, software and trade marks - is and remains the exclusive property of Longaevitas Health (Pty) Ltd or its licensors. Longaevitas grants you a personal, limited, non-exclusive, non-transferable and revocable licence to access and use the outputs of the Services for your own personal preventative healthcare purposes. You may not copy, reproduce, modify, distribute, publish, commercialise or create derivative works from the Services or their outputs without the prior written consent of Longaevitas, save where such use is permitted by law.

19. TELEHEALTH CONSENT

I consent to the conduct of my care through asynchronous telehealth, including: remote consultations; written and electronic communication; virtual review of my Health Assessment, body composition and biomarker information by the Practitioner; digital delivery of VitaPrints, reports, referral letters, supplement guidance and other documents; and the use of electronic signatures within the meaning of section 13 of ECTA. I understand that asynchronous telehealth has limitations relative to in-person care, including the absence of physical examination, and that the Practitioner may decline to issue a recommendation, or may require me to attend an in-person consultation, where in the Practitioner's clinical judgement that is appropriate. The HPCSA ethical guidelines on telemedicine apply to my care.

20. MEDICAL CONDITIONS REQUIRING IMMEDIATE CARE

I confirm that at the time of completing this Consent I am not experiencing, and am not aware of any active suspicion of, symptoms that require emergency medical assessment, including but not limited to: chest pain or pressure; signs of stroke (facial droop, arm weakness, speech disturbance, sudden severe headache); loss of consciousness or fainting; acute severe shortness of breath; major trauma; severe or uncontrolled bleeding; seizures or convulsions; acute confusion or disorientation; suicidal thoughts or thoughts of harming myself or others; or any other rapidly worsening medical condition. If at any time after completing this Consent I develop any such symptom, I will immediately seek emergency care by telephoning 112 from a mobile telephone or 10177 from a landline, or by attending the nearest emergency department, and I will not wait for or rely upon any communication through the Services.

21. FINAL CONSENT CONFIRMATION

I confirm that: (a) I have read every section of this Consent; (b) I have understood the information set out, or had it explained to me, in a language and form I understand; (c) I have had the opportunity to ask questions and to obtain independent advice if I wished; (d) I provide my consent freely and voluntarily, without coercion or undue influence; (e) I understand the risks, limitations and uncertainties associated with the Services as set out above; (f) I have read, understood and agree to the Longaevitas Terms of Service and Privacy Policy as currently published; (g) I confirm that the full legal name I have entered below is mine, and that affixing it by typing it into the signature field constitutes my electronic signature for the purposes of section 13 of ECTA, with the same legal effect as a handwritten signature; and (h) I authorise Longaevitas to retain a timestamped, immutable record of this Consent, together with the date, time, version, IP address and device information of submission, as part of my clinical record.
